Immunization and Physical Exam Requirements
No student will be permitted to attend class, participate in sports, or perform clinical assignments until health forms are submitted and approved.
All students born after January 1, 1957, are required by New York State law to demonstrate proof of immunity against measles, mumps, and rubella (MMR). Proof of immunity is defined as two (2) doses of MMR OR two (2) doses of measles and one (1) each of rubella and mumps. In addition, a signed meningitis disclosure form is mandatory. All students living in campus housing must show proof of having received at least one (1) dose of meningococcal meningitis vaccine.
Program-specific health forms are sent to accepted students when they make their tuition deposit. Forms may also be accessed here:
Traditional First Year/Transfer/ImpactU
Please note that due to COVID19, no physicals will be conducted at the Student Health Center until further notice.
All students except those in Accelerated Degree Programs or Graduate Programs are required to carry health insurance that will be accepted by medical providers in the State of New York. Students without health insurance, those who carry out-of-state Medicaid, and International students are required to purchase a policy through the College for a specific premium established each year. Verification of health insurance is required annually of all students.
All students will see a charge on their bill for the annual college-offered health insurance. If you are a domestic student covered by your own policy accepted in the State of New York, you may waive the college-offered health insurance at the website below and the charge will be removed.
Students who choose to enroll in the college-offered health insurance must indicate that choice at the website below. An enrollment package will be sent to you with instructions for enrollment. The charge on your bill does not mean that you have been enrolled. Please follow the instructions in the enrollment packet.
If you fail to waive the college-offered health insurance by the deadline (August 1 for fall semester; January 15 for spring semester), the charge for the premium will remain on your bill. Students who waive the college-offered health insurance must provide proof of valid insurance in their health forms.
It is a mandatory institutional policy that International Students purchase the college-offered health insurance and their accounts will be billed accordingly.
NO EXCEPTIONS WILL BE MADE.
